80% of older people discharged from the ED have at least 1 unrecognized geriatric problem such as delirium or dementia. Admitting these patients is not a solution to the problem, as it can cause delirium, functional decline, etc. Safe discharge planning is necessary to decrease unfavorable outcomes.
